Subject: Two kinds of attachments in emails - one at the top of the email under a tab, and the other with an icon within the body of the email. How does this work? |

I've been using Lotus Notes for awhile now, and I guess you can have two different kinds of emails with attachments: 1. You can have the attachment in the header of the email with a collapsible "1 Attachment" notification, or 2. You can have the attachment icon located within the body of the email. So if you've got a lot replies in a large thread, it's still easy to see who sent which attachment.
So, I have two questions:
1. I have been looking into writing MIME emails using sendmail, similar to the following:

How do you create the emails with the attachment icon located within the body? I've only been able to create the kind where the attachment is in the header.
2. When I receive an "attachment in the collapsible header"-type email (type 1), I go to File -> Print Preview, and the "To:" and "From:" appear on the top of the page which is good, but the next part of the email (the collapsible "1 attachment" header) appears exactly halfway down the page. So after the "To:" and "From:" block, there's this whole ton of empty space until the header and email body appear. Why is this empty space there, and how do I remove it?
